Are you a 104 member who only works storm, so you do not qualify for the current FR Program? Please call us, there is a storm allotment available. You must be regularly employed for storm work in the area with a participating contractor and have a minimum of 450 hours contributed to the fund in the prior 12 months.
HOW MUCH IS COVERED: $400.00 in a TWO-YEAR PERIOD. Your two-year period is determined by the year in which you order/submit your first pair of boots. The boot program will still run in a two-year calendar cycle starting when you are credited and will reset January 1. You are either in a 20/21 cycle or 21/22 cycle. If you are unsure when your boot allotment will renew you can look in your Tyndale account or call OSHE to verify.
WHO IS ELIGIBLE: You must be CURRENTLY working in the jurisdiction AND:
Members of Local 104: and have 3 FULL TIME months REPORTED in the past 12 months.
Travelers and Non-members: worked the past 12 consecutive months OR 2,000 hours.
WHAT IS COVERED: Boots MUST meet ALL of the criteria in order to be reimbursed: Must meet ASTM 2413-05 Standard by having an impact rating of 75, a compression rating of 75, and an electrical hazard (EH) rating, 6 inches or higher for ankle support. Must have a Steel-toe OR a Composite-toe and appropriate sole/heel for climbing wood poles.

HOW TO GET REIMBURSED: If you still wish to be reimbursed for boots you purchase elsewhere you may submit by fax or e-mail a CURRENT (less than 6 months old)/ DETAILED receipt (Must have make and model number and show the price paid for the boot, the store name and date) and complete the BOOT REIMBURSEMENT FORM that is available in your Tyndale account. They will reimburse you directly, there is a $15.00 fee that will be deducted from your allotment should you choose to be reimbursed. WHO IS ELIGIBLE: : a member in good standing with Local 104 for at least 1 year and have worked 3 FULL TIME months of the past twelve months. YOU DO NOT NEED TO BE CURRENTLY EMPLOYED.
HOW TO ACCESS: You may use any MA recognized training school. Once you have completed your training submit a copy of your certificates and a DETAILED RECEIPT and you will be reimbursed for up to $50.00 per license endorsement (1B, 2A = $100.00). We will not reimburse more than the amount paid and we cannot reimburse on a receipt that does not show the amount paid per course. This is a reimbursement program so you will be reimbursed upon submitting a valid CDL license and a detailed receipt to an approved training school. PLEASE NOTE: YOU MUST BE PRE-APPROVED FOR CDL TRAINING IN ORDER TO BE ELIGIBLE FOR REIMBURSEMENT.
WHO IS ELIGIBLE: A member in good standing with Local 104 for at least 1 year and have worked 3 FULL TIME months of the past twelve month or at the Safety Director’s discretion and approval.
HOW TO APPLY: Forward a copy of your CDL Permit, DOT Medical card, and contact information to the Safety Department in MA or NH and we will let you know where you are on the list and contact you once you have been approved. YOU DO NOT NEED TO BE CURRENTLY EMPLOYED.
This is to inform you the U.S. Department of Transportation (DOT) drug testing program and the IBEW Local 104 OSHE (Non-DOT) drug testing program now require testing for four semi-synthetic opioids (i.e., hydrocodone, oxycodone, hydromorphone, oxymorphone). The change became effective January 1, 2018.
Beginning January 1, 2018, in addition to the existing DOT/Non-DOT drug testing panel (that includes marijuana, cocaine, amphetamines, phencyclidine (PCP), and opiates), you will also be tested for four semi-synthetic opioids (i.e., hydrocodone, oxycodone, hydromorphone, oxymorphone). Some common names for these semi-synthetic opioids include OxyContin®, Percodan®, Percocet®, Vicodin®, Lortab®, Norco®, Dilaudid®, Exalgo®.
If you test positive for any of the semi-synthetic opioid drugs, then as with any other drug test result that is confirmed by the laboratory, the Medical Review Officer (MRO) will conduct an interview with you to determine if there is a legitimate medical explanation for the result. If you have a valid prescription, you should provide it to the MRO, who will determine if the prescription is valid. If not, the MRO will report the result to your employer as ‘positive’.
If a legitimate medical explanation is established, the MRO will report the result to your employer as a ‘negative’. Prior to the MRO reporting your information to a third party you will have up to five days to have your prescribing physician contact the MRO. You are responsible for facilitating the contact between the MRO and your prescribing physician. Your prescribing physician should be willing to state to the MRO that you can safely perform your safety-sensitive functions while taking the medication(s) or consider changing your medication to one that does not make you medically unqualified or does not pose a significant safety risk. If the MRO believes there is still a safety risk after talking to your doctor or after 5 business days (whichever is shorter), the MRO will report the potential safety issue, and you will be directed to have an evaluation with an independent physician. You will not be allowed to work in a safety sensitive position until we receive the results of your fitness for duty examination. NOTE: You will be required to submit to a NEW DOT physical, and a letter will be sent to your employer for the time that you are taking the prescription. If you are taking any prescription medications, consider this to be a notice to have a conversation with your prescribing physician to discuss your safety-sensitive work. Be proactive in ensuring that your prescribing physician knows what type of work you currently perform. This is important information for your prescribing physician to consider when deciding whether and what medication to prescribe for you. It is important for you to know whether your medications could impact your ability to safely perform your job. It is also important that the person who is issuing your DOT medical card be informed of any prescriptions you are currently taking.
As it has been the requirement in the past, when we receive a ‘positive’ drug test result, we will immediately remove you from performing safety-sensitive functions and provide you with a Substance Abuse Professionals (SAP) available in your area.
